form demo fixes

See form.html demo. Let's do some of the suggestions from Alex's mail:

• make form fields same length
• get horizontal alignment better so that "IT" lines up with "high school"
• mysteriously, tan boxes extend 5px farther to the right than blue header boxes

Probably not going to change:

• labels to be right aligned
• * character for required-ness

Tickets already exist for:

• size of input fields (see #6326)
