Dojo CCLA has incorrectly labeled fields

Description

The Dojo CCLA asks for "Full Name" where it probably should be asking for "Company Name".

For reference, the Apache CCLA has the correct fields.

I need to work with Dylan here to ensure we have accurate text as well. The CLA's will no longer live on Dojotoolkit site, but rather on Dojo Foundation site. Need to eliminate references to old CLA, perhaps even version them?
